Improved clinician efficiency, enabling follow-up for up to 200+ patients remotely, compared to 30-40 previously.
Clinicians needed a way to monitor recently discharged or chronically ill patients without frequent in-person visits. Existing processes were time-consuming and made it difficult to track multiple patients efficiently.

Approach
I interviewed nurses and clinicians to build user personas and identify key pain points. With these insights, I created wireframes and iterated with the team to meet clinical needs.

Through collaborative prototyping and feedback sessions, I refined the dashboard’s layout to highlight urgent patient alerts and administrative tasks. The final design allowed clinicians to quickly see which patients needed attention.
